1. Nature & Purpose of Psychological Services
Psychological services offered by licensed professionals at ADZ FeRaL include individual counseling, adolescent guidance, couples therapy, psychometric evaluation (IQ and personality assessments), stress management debriefing, and specialized psychological evaluations.
You understand that psychological interventions require active client engagement and honesty. While therapeutic interventions are clinically proven to improve mental wellness, coping capacities, and self-awareness, specific psychological outcomes or cures cannot be guaranteed.
2. Confidentiality & Its Legal Exceptions
Information shared during counseling, therapy, and testing is held in strict professional confidence under the Code of Ethics of the Professional Regulation Commission (PRC) and the Philippine Mental Health Act (RA 11036).
Mandatory Exceptions to Confidentiality under Philippine Law:
- Imminent Danger to Self or Others: If a client reveals clear, active intent to commit suicide or inflict severe physical violence on another person, the clinician is legally and ethically bound to intervene and notify emergency family contacts or medical authorities to protect life.
- Child Abuse or Vulnerable Adult Abuse: Mandated reporting under Philippine Child Protection laws (RA 7610) regarding child sexual abuse, severe neglect, or elder exploitation.
- Court Subpoena or Order: Legally valid, binding orders issued by a competent Philippine judicial court.
3. Telehealth & Online Teleconsultation Protocols
When conducting psychological consultations remotely via secure teleconsultation links:
- You agree to join sessions from a quiet, private, secure room where conversations cannot be overheard by unauthorized third parties.
- Strict Anti-Recording Rule: Neither the client nor the specialist may record video or audio of sessions without the express, prior written mutual agreement of both parties.
- You acknowledge that teleconsultation depends on telecom and internet connections. If a technical disconnect occurs, the specialist will follow established phone or SMS re-connection procedures.
4. Cognitive & Psychological Assessment Terms
For clients undergoing IQ, BPI, or custom standardized tests:
- Assessment test items are proprietary, copyrighted psychometric instruments. Copying, photographing, or publishing test items is strictly prohibited.
- Automated OMR bubble scoring and digitized results are subject to clinical review and interpretation by a licensed psychologist before final evaluation reports are validated.
- Test reports are confidential medical documents intended strictly for the client, authorized school, or requesting clinician.
5. Voluntary Client Affirmation
By ticking the consent acknowledgment on this platform, you certify that:
- You have read, understood, and voluntarily agree to this Informed Clinical Consent framework.
- You have had the opportunity to ask questions regarding clinic policies, fees, and confidentiality.
- You understand that you may withdraw from psychological services at any time, subject to settling any outstanding professional fees for completed sessions.
